Transforming Lives multi-agency panel.
The purpose of the Transforming Lives panel is to create a bespoke resource to focus on the support needs of adults, families, young people and children to prevent, where possible, escalation into statutory services and provide a range of step down options into universal provision. The process will enable professionals to collectively agree a multi-agency holistic package of support for adults, families, young people and children which will lead to a faster response and more effective step down to universal provision through partnership working and effective signposting.
What Transforming Lives panel expects from professionals making the request for support on behalf of the family or adults?
To explain what Transforming Lives panel is to the family/adult.
- To ensure the service users has sight of and understands CONSENT & DATA SHARING INFORMATION as below.
- Transforming Lives, Strengthening Communities is registered under the current UK Data Protection legislation as part of the Blackburn with Darwen Borough Councils registration for the purpose of processing personal data in the performance of legitimate business.
- In order to provide you with a service, lead professionals from a range of different organisations need to collect personal data for correspondence purposes and specific service provision.
- Your personal data is safeguarded by the Human Rights Act 1998 and the current UK Data Protection legislation. This means that this information cannot be shared or given to anyone without your consent.
- This information may need to be shared with a range of agencies and/or services including: Health, Social Services, Housing Services and Providers, Police and Fire Services, Domestic Abuse Services, Substance Misuse Services, Young Peoples Services, Probation and HM Prison Services, Youth Justice Service, Health Visiting and School Nursing Services, Education Services, the Department for Work & Pensions, the Department for Communities & Local Government, Public Health and the Families Health & Wellbeing Consortium.
- You can refuse for all or part of this information to be shared. The only exception to sharing information without prior consent is if the case is an anti-social behaviour case. Under Crime and Disorder legislation we do not require the consent of the perpetrator in order to discuss information.
To share with the family/adult the Transforming Lives Service User agreement, below, so that they are clear of their commitment and the commitment from the partners offering support.
To discuss what the family/adult feels they need support with and to complete the form with them. Consent must be given and indicated on the form, this can be verbal consent.
To discuss the needs of all family members residing in the household, and these to be included on the Request for Support Form. The support requested should consider the needs of all family members, encourage a whole family approach. If no support needs are identified for individuals in the household please indicate no support required.
If the needs of the family are not made clear and identified in the Request for Support Form the request will be returned to the referrer for more information, delaying access to support for the family/adult.
To check with the family/adult that the information provided in the form is correct and up to date, especially contact numbers.
For families, if there is a Child and Family Plan (CAF) in place, the lead professional will be identified and co-ordinate support, and the plan, with the family and the partners. Partners will maintain regular contact and feedback to the CAF lead professional.
For adults, if there is an adult social worker supporting they will co-ordinate the support and the plan with the adult and partners. Partners will maintain regular contact and feedback to the social worker.
If there is no CAF in place for families or no adult social worker supporting adults - where possible, if the professional making the request for support is maintaining regular contact with the family or adult, the professional is also requested:
- To coordinate support with the family/adult and the partners and encourage engagement with the partners.
- To ensure the family/adult and the partners have a clear plan and distinct roles within the plan.
- To track progress made and alert partners to any issues or concerns arising.
- To capture outcomes and report to Transforming Lives, when support comes to an end, the difference the interventions, from all partners supporting the family/adult, has made to the family/adult. If support is ongoing TLadmin will request quarterly updates.

Each request has a unique reference code, Case ID, this is provided in the Transforming Lives Panel Outcome e-mail, please quote this unique reference in your correspondence so we can identify the family/adult.
